IV. Pharmacology and 

Toxicology

There is growing science about the pharmacology and toxicology of transfer of alcohol and recreationally-used medicinals into human milk. However, mainstream/online articles inaccurately describe the biology, and conflate true risks to the infant. Parents who engage in “adult use” of regulated or quasi-legal substances, are often shamed-and-blamed for their behaviors.  They are reluctant to seek information from healthcare providers. They end up using tactics and techniques advised by profit-driven companies or celebrity bloggers, that unnecessarily deprive the ease and long-term health benefit of direct breast/chestfeeding.  Some families end up introducing formula, or even weaning, much sooner than planned. This session will review the impact of partying on human milk, and the ethics of advising partying parents in your care. The ethical obligation to provide patient-centered, evidence-based information and support will be explored. The conflict-of-interest inherent in “commercial determinants of health” (where parents make decisions based on heavy marketing, not healthcare needs) will be explained. This session is eligible for E-CERPs and covers material from three sections of the IBLCE Detailed Content Outline: IV-Pharmacology and Toxicology, V-Psychology, Sociology and Anthropology, and VII-Clinical Skills/C-Ethical and Legal Issues.

Liz Brooks is a private practice International Board Certified Lactation Consultant (IBCLC) and licensed lawyer. She works as a postpartum per diem IBCLC at three BFHI-designated hospitals, educator/speaker, and lactation expert witness.  Lactation helpers (like IBCLCs) struggle with ethical, legal, and moral concerns in their everyday work settings. With plain language and humor, Liz shares tips for immediate use, to protect both the family and the clinician. She authored the only textbook on legal and ethical issues for the IBCLC, and writes extensively for other textbooks, blogs, and peer-reviewed journals. Liz is a popular international conference speaker, using wit and wisdom to share pragmatic suggestions for anyone who supports lactating and human milk-using families. Liz self-identifies as a cisgender hetero white woman with unearned privilege, uses she/her/hers pronouns, and advocates for inclusive care and language.

Labor and Delivery/Postpartum RN at Abington Hospital from 1991-2002. Home care RN from 2002-2021, visiting new mothers and their babies and providing lactation consultations. Returned to inpatient nursing as full time IBCLC on postpartum unit at Jefferson Abington Hospital where approximately 4500 births occur.
